Personal Details
Please enter your date of birth and name in English. Make sure to write your name exactly as it appears on your passport, as this is the name that will appear on your diving certificate.
Double-check that you have entered your correct date of birth, including the year (people sometimes mistakenly enter the current year). Once verified, click Submit.
Verification and expansion of personal details
You will now arrive at the page with the details you filled in. You should verify the details and you can add a middle name (if there is one and you want it to appear - not recommended because it makes it difficult to search in the future)
Also complete the details gender, date of birth and others

The address must be filled in for the delivery of the certificate
In the second line State/Province, fill in a region in the country, for example "hadarom" (the south)
You must fill in a zip code, if you don't know it you can search for it by address on the post office website - to find a zip code click here

*In case some of the details are unclear or not filled in properly, a correction message will pop up and only after that you can go to the next page

A page will now open for attaching a photo. A face photo (passport) with a light background must be attached
Please note - if you are using a mobile phone, the picture must be taken with the phone sideways, it cannot be rotated afterwards
1. Click on choose file and find it on your computer > an expanded page will open
2. Position and expand the square on the face
3. Cut the image (crop) and press save
